Rather, it is intended to be an alternative to hospital admission that still provides the same access to healthcare services that an inpatient admission would. Hospital-at-home should not be used as a means to restrict a patient’s access to acute specialty care. What levels of care should not be considered hospital-at-home? Traditional home care should not be considered its equivalent, nor should enhanced primary care, ongoing chronic condition management, intermediate care, or custodial daycare. 2Ī hospital-at-home program involves the use of a multidisciplinary team (as would be used in a hospital) to manage the patient’s condition, and it provides access to many of the same interventions (i.e., IV fluids, oxygen, etc.) that may be required to treat the patient. Under that program, hospitals could transfer patients to other healthcare-providing facilities (i.e., ambulatory surgery centers, inpatient rehabilitation hospitals) but also hotels, skilled nursing facilities (SNFs), or even patients’ domiciles with the hospital still receiving Medicare payments for the care provided. 1 Even prior to the COVID-19 pandemic, hospital-at-home programs were expanding, and the trend accelerated when the Centers for Medicare & Medicaid Services (CMS) launched its “Hospitals without Walls” initiative in March 2020. Hospital-at-home is a healthcare model designed to provide acute-level services in a home environment that patients would normally receive in a hospital setting. However, another possibility for acute care treatment has been gaining traction in recent years, and its use has accelerated with the arrival of the COVID-19 pandemic. These decisions can be supported with clinical evidence from various tools such as MCG’s Rapid Review guidelines or assistive artificial intelligence (AI) platforms such as Indicia for Effective Focus. For acute care in the U.S., additional decisions often have to be made regarding the most appropriate level of care, such as choosing between Observation Care status and full inpatient admission. Healthcare professionals have the immense responsibility of delivering the best possible care to a patient based on the symptoms being presented as well as the clinical needs of the diagnosis.
